THE CORRELATION OF HBA1C AND DIABETIC RETINOPATHY WITH CORNEAL ENDOTHELIAL PARAMETER IN PATIENTS WITH TYPE-II DIABETES MELLITUS

Abstract
Introduction: To understand the correlation of HbA1c and diabetic retinopathy to central corneal thickness and corneal endothelial cell density in patients with type-II diabetes mellitus.
Methods: This cross-sectional study was conducted from July 2023 to February 2024 at Saiful Anwar General Hospital. Corneal endothelial parameter were examined using non-contact specular microscope NIDEK CEM-530. Patient groups are diabetic mellitus patients with diabetic retinopathy or diabetes mellitus patients without diabetic retinopathy.
Result: We included 101 patients with type-II diabetes mellitus, most of which (67.3%) had diabetic retinopathy. There is significant correlation between HbA1c and endothelial cell density (r=-0.340, p=0.000), but not between HbA1c and central corneal thickness. Diabetic retinopathy was not correlated to both central corneal thickness (p=0.546) and endothelial cell density (p=0.081). We also found that there were correlation between HbA1c and hexagonal (r=-0.294, p=0.003) and significant correlation between diabetic retinopathy and hexagonal (r =-0.333, p=0.001) and coefficient variant (r=-0.324, p=0.001).
Conclusion: This study found out that the higher the HbA1c, the lower corneal endothelial cell density but no correlation was found to central corneal thickness. Diabetic retinopathy did not correlate to either central corneal thickness or corneal endothelial cell density.
